You can fill the form online by using the form directly below by clicking on the “T”, then clicking on the entry line, a box will appear, you then type directly into that box, moving through the form to fill out all the spaces and print it out. Please note – some Apple products do not support this option.
Or you can print out a copy of the form using the button at the bottom of the page and then hand write in the blanks.
Either way, please bring a copy of the form already filled out to the conference.
